Services Provide
Primary Care
Primary Care is provided using the VA’s Patient Aligned Care Teams (PACT) model of care, which emphasizes personalized, proactive and patient centered health care to address personal health needs. Each patient who needs ongoing care for preventive health and chronic medical conditions will be assigned a single primary care provider, either a physician, nurse practitioner or physician assistant. Primary care is provided by a multidisciplinary team including physicians, nursing professionals, health technicians, social workers, clinical pharmacists and administrative support staff.
Women's Health Primary Care
Women's Health Primary Care includes gender specific preventive care and office management of common gynecological problems. These services are provided at all VISN 1 medical centers and community based outpatient clinics.

Telephone Assistance
Ready access to the primary care team is available by telephone and provides advice, education and follow-up through telephone visits. Telephone care during normal working hours is provided through a clinical call center based at each medical center. Telephone advice is also available to answer clinical questions on weekends, holidays, evenings and nights.
Management Oversight of Community Based Outpatient Clinics (CBOCs)
The main focus of CBOCs is primary care, and the majority of staffing in CBOCs consists of primary care staff. Services other than primary care provided at CBOCs remain under the oversight of their respective service lines.
